We will make transfers on the following basis:
If a transfer date is a non-processing day for North Central Bank then the transfer will be made on the first processing day after the scheduled transfer date.
**A voided check must be submitted to North Central Bank in order to set-up your EFT/ACH Request.
By signing below, I authorize North Central Bank to initiate debit/credit entries via Electronic Funds Transfer through the Automated Clearing House (EFT-ACH). I understand and agree that I must allow North Central Bank fifteen (15) calendar days to process and initiate the EFT-ACH deposit transfer/loan payment.
I understand and agree that if my account at the depository financial institution listed above does not have sufficient funds to make the transfer, North Central Bank will not be responsible or liable for any penalties or charges assessed by any other financial institutions as a result to such insufficiency. North Central Bank will attempt to make the EFT-ACH transfer up to two (2) times.
If my loan is a variable rate mortgage and the interest rate changes, I understand and agree that my EFT-ACH payment will be updated to reflect the change(s), per my loan agreement. If my loan mortgage loan has escrow and the escrow payment changes, I understand and agree that my EFT-ACH payment will be updated to reflect the change(s), per my loan agreement.
I understand that this authorization may be terminated by me at any time by written notification to North Central Bank, and that I must allow fifteen (15) calendar days after receipt for termination to be effective.
By submitting this application, I/we authorize a consumer credit report and verify the statements in this application. Furthermore, I/we agree to be bound by the terms and conditions of the debit card, and the electronic funds transfer disclosure, copies of which will be mailed to the applicant if a card is granted. Receipt of terms and conditions, and disclosure, and acceptance of such terms will be conclusively presumed by use of the card. If this is a joint application, the undersigned shall be jointly and severally liable for any and all debit card transactions. Both parties must sign if a joint account is desired.
